If mailing, please include the facility name, address and “Attn: Health Information Management” on your mailing envelope. The California Medical Association recommends physicians keep records for at least ten years from the last date the patient was seen. Facilities and contact information are listed below. In California, physicians must notify patients in advance of closure of the practice, and are still responsible for safeguarding records and making sure they are available to patients. Fax, email or mail the completed Authorization form to the IU Health facility from which you are requesting records.Completing the form in its entirety will help ensure timely processing.
